WELCOME TO DIET OF HOPE INSTITUTE

NEW PATIENTS

Please arrive 15 minutes prior to your appointment time with your new patient paperwork completed. If you have not received and completed the new patient paperwork please arrive 30 minutes early. 

The following are needed for your first visit:

•  Photo ID and insurance card(s)
• If you have HMO, Try-Care Prime or AHCCCS insurance you will need to bring a referral from you PCP
• A list of current medications and dosages
• If applicable, a copy of any recent bloodwork. If there is no recent bloodwork (within 3 months) labs may be drawn at your appt.

YOUR FIRST APPOINTMENT WILL BE 1 1/2 HOURS FROM START TO COMPLETION

PAYMENT OPTIONS

We accept most major insurances, AHCCCS plans, and Medicare. Bring  your health insurance card and a picture ID to your appointment. 

Diet of Hope Institute also offers a cash payment for those who do not have insurance.
